We employ an experienced clinical staff of over 100 highly qualified experts who specialize in providing care to the senior population. Each facility is assigned a psychiatrist, nurse practitioner, and therapist (licensed psychologist and/or clinical social worker). Our complete team works with facility staff to deliver the highest-quality care.

Assessment
At the request of the patient or medical providers, PGS will provide a comprehensive assessment of the patient, recommend a treatment plan, and refer the patient for necessary ongoing care.
Medication Management
PGS prescribers monitor and manage psychiatric medications for patients. We provide reports and data to help facilities manage population health metrics and maintain compliance with CMS regulations. Gradual Dose Reduction (GDR) is a key element of care for PGS clinicians. Whenever possible, PGS clinicians are looking to discontinue unnecessary medications to reduce adverse effects.
Psychotherapy
PGS provides psychotherapy through licensed psychologists, social workers, and nurse practitioners if medically necessary. Services include individual counseling and family therapy.
Crisis Intervention
PGS clinical staff are available 24 hours to assist with managing crisis behaviors via telehealth or in-person, if necessary. Our 100+ providers ensure consistent coverage. When an issue arises, you will be able to reach to them on their personal cell phone for their expertise, and (if necessary) with hospital admissions for the patient in crisis.
Hospital Placement
PGS providers have relationships with many hospitals and will assist facilities to identify the most appropriate placement when hospital admission is necessary. PGS also assists with referrals to other psychiatric facilities when requested by the nursing home or a resident’s family.
Staff Training
PGS professionals provide facility staff with seminars on a wide variety of clinical topics, including behavior management and the psychopharmacological aspects of specific medications. Topics are tailored to the facility staff and their patient population.
Referral Process
PGS has a streamlined referrals process to ensure we promptly see patients where the facility has identified behavioral, mood, or medication concerns.
Program Consultation
PGS collaborates with each facility to ensure compliance and quality. In addition to attending regular quality assurance and pharmacy meetings, PGS will review clinical operations programs upon facility request.
Compliance
PGS provides clear, consistent data for Quality Assurance/Performance Improvement (QAPI) reports and to improve coordination of care across each facility.
